Job Summary:

Advances an integrated community health strategy that aligns community health, population health and care coordination leadership, resources and partners. Develops, coordinates and evaluates projects related to community health activities that align our community involvement and relationships with our Mission. Works with community partners to coordinate and improve the health for low income and vulnerable communities. Oversees the Community Health Needs Assessment (CHNA) and Community Health Implementation Plan (CHIP) process as well as the community benefit reporting.

Job Requirements:

Education and Work Experience:

  • Bachelor’s Degree in business, public health or related field or equivalent combination of education/related experience: Required
  • Masters Degree in public health, healthcare administration or related discipline: Preferred
  • Five years relevant experience: Preferred
  • One years leadership experience in healthcare, education or non-profit: Preferred
  • Demonstrated experience in community and population health management and experience building effective relationships with leadership internally and key stakeholders outside of the organization: Preferred

Essential Functions:

  • Assists in the development and implementation of a comprehensive community health strategy and program in support of the systems strategy, vision, values and goals. Identifies and implements community health best practices from across the system. Represents organizations community health interests with external parties and develops partnerships with community resources.
  • Oversees CHNA, CHIP and CB reporting systems and processes. Ensures compliance with 501r regulatory requirements.
  • Facilitates collaboration and alignment with population health initiatives by implementing strategies to support defined populations, developing community capabilities, systems and processes to manage super utilizers. Works with clinical leaders to align clinical care with community resources to impact social determinants.
  • Creates alignment between operational realities, strategic initiatives and community opportunities. Works with marketing, chaplains, employee wellness, volunteer coordination and other departments as appropriate to align strategic initiatives. Identifies opportunities for employee engagement in the community.
  • Performs other job-related duties as assigned.
