Job Summary

To perform day-to-day nursing activities of the facility in accordance with current federal, state, and local standards, guidelines, and regulations to ensure that the highest degree of quality care is always maintained. To complete tasks under the supervision of the Director of Nursing. To assist in maintaining a physical, social, and emotional environment that fosters the best interest of the residents and their families. To promote person-centered care for each resident.

Supervisory Responsibilities:

  • Supervise Resident Assistants.

Duties/Responsibilities:

  • Evaluate the residents physical and emotional status.
  • Observe for symptoms of illness/injury and changes in condition.
  • Take and record TPRs, blood pressures, weights, etc., as necessary.
  • Notify the residents attending physician and family when there is a change in the residents condition.
  • Consult with the residents physician in providing the residents care, treatment, rehabilitation, etc., as necessary.
  • Arrange for diagnostic and therapeutic services, as ordered by the physician, and in accordance with established procedures.
  • Obtain sputum, urine, and other specimens for lab tests as ordered.
  • Provide direct nursing care and administer medications and treatments per current physicians orders and according to the facilitys policies and procedures.
  • Ensure medication administration record reflects all medications administered or refused.
  • Ensure treatment administration record reflects treatments performed or refused and assessments of wounds.
  • Document progress notes on residents as needed.
  • Assume responsibility for proper use and care of equipment used in the treatment of residents.
  • Ensure that all nursing care is provided in privacy and that nursing service personnel knock before entering the residents room.
  • Make periodic checks to ensure that assigned tasks are being properly performed by resident assistants.
  • Use personal protective equipment whenever performing procedures that involve exposure to blood or body fluids.
  • Ensure that assigned work areas (i.e., nurses stations, medicine preparation rooms, etc.) are maintained in a clean and sanitary manner.
  • Ensure that an adequate stock level of medications, medical supplies, equipment, etc., is maintained in the nursing office at all times to meet the needs of the residents.
  • Notify Director of Nursing when a resident is transferred to the hospital.
  • Report complaints and grievances made by the resident or family to the Director of Nursing.
  • Complete accident/incident reports as necessary.
  • Cooperate with other resident services when coordinating nursing services to ensure that the residents total regimen of care is maintained.
  • Attend departmental and staff meetings, in-services, and on-the-job training programs, etc., as scheduled or directed.
  • Perform other related assignments as directed by the Director of Nursing or the Executive Director.
  • Willingness to cross-train and assist where needed.
  • The employee is required to maintain all HIPAA information in confidentiality.

Required Skills/Abilities:

  • Must be knowledgeable of nursing practices and procedures.
  • Must possess the ability to make independent decisions when circumstances warrant.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.

Education/Experience:

  • Must be a graduate of an approved LPN program.
  • Must possess a current, unencumbered, active license to practice as an LPN in the state of Ohio.

Physical Requirements:

  • Must be in good physical health.
  • Be able to walk to resident apartments as needed.
  • Sit for extended periods of time.
  • Must be able to lift a maximum of 50 pounds.
