Description

The Clinical Liaison position is responsible for creating and maintaining positive customer relationships in order to increase referrals for Home Health, Hospice, Home Infusion, PACE, Transitional Care Unit and other services within assigned accounts. This role includes working with discharge planners, facility staff, physicians, and individuals to assess appropriateness for hospice and home health services, and facilitate the coordination of services and to ensure there is appropriate documentation for compliance with agency and statutory guidelines to manage the transition of the patient from an acute care facility to the patients home or next location of residence. Consultative functions include, but are not limited to: providing consultation and informational presentations regarding end of life care and home health; sharing expertise through in-services on pain and symptom management options; serving as a clinical resource and educator for physicians, facility staff, patients, families and community on Home Health or other issues in various settings and facilities.

Required Qualifications:

  • Education sufficient for clinical licensure for discipline (e.g., LPN LVN, Registered Dietitian) or Bachelor’s Degree in Social Work
  • Current state professional license or certification appropriate to discipline upon hire.
  • 3 years experience in clinical discipline.

Preferred Qualifications:

  • Masters Degree in Nursing
  • Bachelors Degree in Applicable clinical discipline (e.g., Respiratory Therapy).
  • Liaison, marketing or sales experience.
  • Health care industry experience.
  • Documented prior successful sales account medical management experience.
  • Epic EMR experience.
